新型コロナウイルス感染症の影響より食生活に著しく支障をきたすほど家計が悪化している学生の
食生活を支援するため、食生活支援プログラムを実施します。
支援を要望する学生は、下記の申請方法により、４月１４日（金）【必着】までに申請書類を提出してください。

１．申請書類：医学部教育研究支援基金(KMS-FUND) 食生活支援プログラム
申請書及び同申請書の指定する収入証明書類
２．申請書類の提出先及び提出方法：医学研究科教務課に設置する提出ボックスに、
封筒に封入した申請書類１部を提出すること。
３．申請期限： 令和４年４月１４日（金）【必着】

Dear all students of the Graduate School of Medicine and Faculty of Medicine,
From the Office of Student Affairs,
Graduate School of Medicine and Faculty of Medicine of Kyoto University will implement the Food Assistance Program
under the support of KMS-FUND to support supplement the diets of students whose financial situation has been in such
a severe condition that remarkably affect their eating habits negatively due to the impact of Novel Coronavirus (COVID-19).
Those who wish to have the support, please submit the application documents by the following application method by Friday 14th April.
Overview:
1. Application documents：
Application form for the KMS-FUND Food Assistance Program and documents of income certificate designated in the application form.
2. Where to apply and how to submit：
Please submit a set of applications after putting it inside an envelope and seal it in the established box at the Office of Student Affairs Division.
3. Application deadline：
Application documents must arrive no later than Friday, 14th April.
